If you want the steam to work, yes. Without a panel closing that gap the vapor climbs straight out the top and the shower never holds temperature. The transom is a fixed or venting piece of tempered glass sitting between the door head and the ceiling. Venting versions let you open the top afterward so the enclosure dries out instead of staying damp all day.

That is mostly a templating question. We map the ceiling and the walls first, then cut the glass so the gaps are tight enough for the seals to actually do their job, and we detail every jamb and joint. Where an older wall leans, we account for it in the panel rather than forcing the glass flat against it. Sloppy measuring is what makes steam enclosures leak.

Usually. Plenty of older bathrooms have modest ceiling height, which is fine: we simply build the glass shorter and keep the transom in proportion so it still looks intentional. What we check is whether the ceiling holds a consistent height across the enclosure and whether the door still has clean swing room. Anything that will not work, we flag before you order glass.
